Heparin Dosage problem! PLEASE HELP!!?
If someone could please help me solve this....and show the steps on how to do it, it would be greatly appreciated!!
Your pt has 500cc D5W with 28,000 U of Heparin at 22cc/hr. Following her PTT, the physician tell you to increase the rate by 250U/hr. How many cc/hr will the pt recieve at the new rate? (gtt factor=60).

The question is really what volume of your solution contains 250 unit of heparin, because that is the extra volume you have to give per hour. Try it this road:
28000 units equivalent to 500 cc
1 unit equivalent to 500/28000 cc
250 units equivalent to 250 x 500/28000 cc
This is a method for calculating ratio which always works. As a sideline, the gtt factor is irrelevant unless you are specifically asked how many drops per minute you have to contribute. As a second sideline, if this were in the real world I would ask the pharmacist to stop making such silly concoctions and put 25000 unit in 500 cc instead. It makes the maths so much easier, and when drug calculation errors are one of the main causes of drug administration errors that is a serious consideration. ALWAYS include the units when you do any calculation. When you cancel where appropriate, if the units vanished are what's required then your setup is probably correct.
First, how many units/ml
28,000U / 500ml = 56 U/ml
How many ml contains 250U
56U/1ml = 250U/x ml
(1ml * 250U) / (56U * xml)
x ml = (1ml * 250U) / 56U (cancel the U and you are vanished w/ ml)
x = ~4ml (actually 4.5ml)
Old rate + new amount = new rate
22cc/hr + 4ml (or cc) = 26cc/hr
PS. An IV of 25,000U/500ml (50U/ml) would make the weighing up and rate much more accurate. Source(s): Pharmacist
