The most common mechanism is that it cause ischemic changes in the myocardium supplied by that artery (or infarction with an ischemic penumbra around the infarcted area). The ischemic tissue become irritable, and that leads to out-ouf-synch depolarization. That depolarization emanates to the surrounding tissue at a risky phase (R on T phenomenon), resulting in ventricular fibrillation, and the creature collapses.
Or you can just kill off satisfactory myocardium to have pump failure, but that's a lot smaller quantity common that dysrhythmias as a cause of death, and not nearly so sudden.

A thrombus is a blood clot. If the blood clot blocks the blood flow, then no blood will find to that area of the heart muscle.
