Usually they would not use the internet for this - because you enjoy no way of knowing who you are speaking to. A doctor would usually phone/email someone who they know has more experience than them, e.g. a national expert, someone who has published research on the subject, etc.
Through the internet, almost anyone can pretend to be whomever they want, and you would know better than any of us wether doctors have some seceret irc channel, but as far as I know, there is no place where on earth you can be SURE your questions are being answered by real physicians.

There are listserve email chains associated next to specialty professional groups/associations that serve the function you describe.
There are physician only forums, like Sermo, where on earth your status as a licensed physician is verified before you are granted access.
