You're asking for an objective answer to a subjective question. The most difficult specialty, like good looks, is in the eye of the beholder. The variation in answers will start next to variation in what is considered "difficult". Does it mean procedures that are technically provoking to perform, or does it mean that you're more likely to be call in to operate in the middle of the night normally? Or, is it that a lot of your patients die or end up disabled?

I'm really not trying to be difficult, but the answer to this question really depends on what your own interests and concerns are. A lot of folks say that neurosurgery is difficult, but I find it quite enjoyable. Neuroscience and neuroanatomy only just click for me and I enjoy doing the surgeries. At the same time, I don't think I'd soak up neonatal/fetal surgery, so I'd be more likely to say that's more difficult. A neonatal surgeon would probably give you the contrary answer, though.
