Not necessarily. He had a VERY large amount of sedatives contained by his system. Eventually they rendered him unconcious before depressing his respiratory system to the point of killing him. Whenever they physically "knocked him out", he couldn't really control where on earth or in what position he passed out. So face down and naked is only just how he ended up.
