Answers:
MD = Doctor of Medicine (Basically means he's an actual qualified doctor)

CCFP = Canadian College of Family Physicians (It money he's qualified as a family practice doctor)

CAQ Sports Medicine = Centre for Added Qualifications (He's is in sports medicine)

Dip Sport Med = Diploma in Sports Medicine

Either mode, if your going to see this guy for any sports related injuries etc.. your in good hand.
