If levobupivaciane is so much safer than bupivacaine why is so much bupivacaine still anyone used within the US?

Bupivacaine is safe, it works, and it's cheap. What's wrong with that combination?
We have a complete slew of active isomers now with patent and price tags that are out of all proportion to the tiny improvements involved. It just doesn't receive sense to spend many times the money per dose across the board when the problems avoided only occur once contained by a blue moon. If there's some particular risk identifiable in an individual patient, that's one item. But to switch in all comers is too much expense for too little gain.
