Small lymphocytic lymphoma and chronic lymphocytic leukemia?

What are the differences between these two diseases? Do they always occur simultaneously? Why are they always combined approaching SLL/CLL, are they the same disease?

They are the same disease. If it is found in the lymph nodes first ie detected clinically by a swollen lymph node and a biopsy is done consequently it is SLL.
If it was detected by a blood test first and testing done on the blood afterwards it is in leukemic phase thus the name leukemia and called CLL.
It eventually is found surrounded by both the lymph nodes and in the blood.
