Vegans and a few others can have a dietary deficiency, but what you're asking more or less (and might want to google) is pernicious anemia. That's generally a deficiency of the intrinsic factor secreted by the parietal cells of the stomach, and that factor is needed to involve the B12 in the diet. What causes the problem with the parietal cell varies, but it's often an auto-immune problem and has some familial susceptibility.
Vitamin B12 deficiency can be caused by several factors.

1) Not plenty dietary Vitamin B12. If you're not taking in enough B12 in your food you will eventually become imperfect.
2) If part of the stomach is removed. The stomach secretes a chemical called Intrinsic Factor that you involve to absorb B12, and the less stomach to make the chemical, the smaller number chemical is made and the less B12 absorption.
3) Pernicious Anaemia. This is an autoimmune condition where your own antibodies attack the cell of the the stomach, killing them and making them unable to produce the Intrinsic Factor, leading to malabsorption of B12.

There are several other cause of B12 Deficiency but those are the 3 main ones. The others include Tapeworm, Blind-Loop Syndrome and damage to the Ileum (which is where B12 is absorbed) among others.

Also, B12 Deficiency will not exact you to bleed to death. Those tend to be more due to lack of clotting factors or platelets. Source(s): Medical Student
