If you are on diazepam, what does it enunciate on your prescription sign?

This might sound like a bit of a weird give somebody the third degree, I know. But I'm working on a play and need to know what the wording might be on a bottle/packet of diazepam, the instructions for taking it, I mean. I know it will differ from doctor to doctor, but if anyone is/was/knows someone who is on diazepam, would you mind posting the wording here?
