You can, but that requires doing two residencies. Most of us, by the time we are in our thirties, are primed to get a real job and practice what we've be training forever for.
Any specialty will give you enough work for two full time jobs. Why would you try to do two unrelateon cloud nineds? So you can be mediocre in both?
Unless, of course, you get tired of one specialty and option to go in a new direction. I know a few docs who own done that.
It would be better to be certified within two complementary specialities, like neurology and biomechanics, rather than the example you specify.
An immunologist could also be an infectious disease specialist, or a public health specialist.
It take many years of training to be board certified in a specialty, so it would not be productive to have two specialities where on earth you are not likely to see similar patient populations. For instance, an immunologist would see people next to infectious diseases, but would generally not see Parkinson's patients, like a neurologist would. It's like apples and oranges.
