What is a "contraction on cortex" contained by regard to the Knees?

I had a knee sprain about 6 months ago within University, and the doctor there only gave me a Tylenol..i fixed to see a specialized doctor and he gave me an MRI...his PA told me that i have this contraction on cortex, and she said its usually caused my trauma, however i hurt my knees standing up and i didnt fall nor get hurt...she said i need to grasp a steroid shot, and then go for some knee surgery...is that really called for? (my knee cap moves around, and there is still fluid on it) and again, whats this contraction on cortex?
