What things could raison d`¨ētre a man within his untimely 40s to suddenly jump bonkers and not restore your health? I plan close to incoherent?

speech, suddenly loosing his train of thought and going quiet, periods of total apathy, sudden bursts of energy where on earth he starts running around and dancing, often without clothes on and not showing any signs of shame..

I would appreciate any design as to the cause..
