Can the doctor write another physical form?

I play football and i needed a physical, so i got one. But the coach didnt ask about it and i forgot about it and presently i lost it, and have no idea where it is (i estimate my mom threw it away on accident). So i was wondering if i could ask the doctor if she could write me another one. Do you think she will make me reward again. I am low on money and my mom wont pay for another one.

The form is part of your record. Your doctor should be able to fax a copy to the coach, no charge.
he should have a copy of the form..ask him to fax it to you or ask if you can come in and return with a copy....it might cost you no more than 5 dollars
You can get another blank form from your coach or perhaps download one from the Internet. Fill out your module of the form, answering the questions about your previous medical history, then post or take the form to your doctor, asking that she fill it out based on her recent exam. If you messages it in, you might include a brief note explaining that you lost the original. Most office won't charge you a second fee.

Be sure to fill in adjectives the appropriate spaces. I think you and your parents must sign a HIPAA form too, nowadays. Sending the doctor a blank form won't cut it, at least not surrounded by my office. Source(s): Retired pediatrician.
