Basically a school physical is to check the things almost you physically that the school could have liability for in exercise or gym class.
That should involve a right long listen to your heart and lungs, a good look in your eyes, at the optic disc, blood pressure, urine check for glucose (to rule out diabetes), check for hernia.
All those things could uncover something that, if near is disease could be harmful when you are exerting yourself.
Other than that, they don't much care.
A check-up and a physical are two different terms for equal exam. Either exam may be complete or limited, depending on the purpose. Part of the process is obtaining your medical history, and yes, we do ask teen agers about sexual behavior, dietary behaviour, smoking and drug use.
