What happen after internship when becoming a doctor of pills?

so complete med school --> internship --> then what happens?
do you purely work in the hospital, but what is your title then?
when does residency occur? as much info. as possible please :D
also is this relevant to australian medical training?

Internship is the first year of residency. When you finish the first year, you move on the the second year, and so on. That's surrounded by the US.

I have no idea how the Aussies do it.
