Answers:
A sliding hiatal hernia (type 1 hiatal hernia) is a disruption of the normal anatomical relationship of cardiac sphincter with respect to the diaphragm. Consequently the cardia of the stomach herniates upward into the posterior mediastinum. This process does not affect gastric bitter secretion, and in fact although most sliding hiatal hernias are asymptomatic the main sequelae to this condition is an increased risk for gastroesophageal reflux disease. An increased risk of heart attack would be unlikely to impossible because ventricular pressures are much much greater than peritoneal or gastric pressures. Furthermore, intrathoracic pressure is low, and will not allow compression of the heart or great vessels by herniating abdominal contents. Type 2, or para-esophageal hernias can be large enough to mete out mechanical compression of lungs and result in respiratory complications. Sliding hiatal hernia does not cause a kink within the throat, the herniation is much lower, and the esophagus remains straight in this condition. Dysphagia can occur from gastric volvulus (twisting) but this is more associated with a type 2 hiatal hernia. Furthermore, this is not like peas in a pod as choking, which involves a blocking of the airways by food. Type one hiatal hernia is a relatively benign condition, and does not require surgical correction unless severe and producing symptoms such as GERD. In such cases surgical correction can be performed, or medical management to suppress acid secretion can be an choice.
