Sounds approaching a very high dose. Are you sure its not 2.5mg? She definitely wants to have frequent bloodwork- a PT/INR every day or every other day. I've be an RN for 30 yrs & never gave anyone more than 15mg (a) a time. Some people do require higher doses, but not 25mg a time! If this is correct & she is having her labs drawn as recommended by her Physician, then perhaps she have something else going on that causes her not to absorb the med. Source(s): RN
25mg is a bit high, but the belief is to get the dose therapeutic for the person. Here's the warfarin protocol:

Usually the upper demarcate is 10mg, but your friend may require more. Most doctors don't follow warfarin initiation algorithms and give dosages that vary by situation and INR values.
