Answers:
1. any college where on earth you can take the classes needed to apply to medical school
2. 20K or more
3. 3-4 yrs college, 4 yrs med school, 3 yrs internal tablets residency, 3-4 years cardiology fellowship, yearly continuing medical education courses after that for the rest of your life
4years of underneath graduate the 4 years of graduate school or medical school to get your doctorate. After that you must do a residency surrounded by Cardiology that will be 4 to 5 years.

The under grad can be in most any thing but best if contained by chemistry.

Pay after all that would be 250k to 500k may be more.
